- Highly detailed description of observed environmental issue. Please include any company or personnel names that were observed to be involved.
- Exact location of occurrence:
- Full addresses
- Coordinates
- Township
- Photographs of the issue.
- Your contact information so that the BCCD may follow up to inquire for further details or directions to the site.
